Frailty Care Co-Ordinator: The term “frailty” or “being frail” is often used to describe a particular state of health often experienced by older people.  A Frailty Care Coordinator supports patients with mild to severe frailty within the community, working with them to create a patient-centred care plan. One of Selby Town Primary Care Network’s coordinators is  Alice – and she can be contacted directly  on 01757 244983 or via the team on 01757 244980.
